Revival · Reporting

If something sat wrong
with you, tell us.

That's the whole bar. It can be something that happened at one of our events, somewhere else entirely, or something you know about a person.

If this is an emergency, call 911. Telling us is not a substitute for the police or the courts. We're a volunteer board, not a legal system, and we'll be honest with you about what we can and can't do.

What happens next: one designated board member reads this. It gets its own file and case number. You hear back with that number at the contact you give us. You decide what happens from there. We will not share your name or what you wrote with anyone you're reporting.

Writing on behalf of an organization, event or venue? Choose that under "I'm sharing this as" and we'll ask you a few different things.

Which organization, venue or community does this relate to?*
Pick one so we know where to look.
In your own words. Include roughly when, a year is fine. As much or as little as you want. This is the only box you have to fill in.
This is the one box we need.
Which of these fit? Check any that do, or skip it.
I'm sharing this as*
Pick one.
Names if you know them. If you don't, describe them: roughly how old, height and build, gender if you know it, hair, anything they were wearing or carrying, their camp or role. Anything that would help us place them.
Does this involve anyone on Revival's board or organizing team?*
Reports come to one designated intake reviewer. If a report involves that person or anyone else on the board, it goes to a different board member, and the person named is kept out of the review. You don't have to hold back here, and nobody on the board sees a report that names them.
Pick one; it decides who reads this.
Does this involve anyone under 18?*
Pick one.
What would you like to come of this?
Check all that apply.
Has this been reported anywhere else?
If this person's name comes up again
Patterns only become visible when someone is keeping track. One story is a data point. Several, from different people, is something we can act on. If this name comes up again, we may want to come back to you. Nothing here commits you to anything, and "no" is a complete answer.

Your name and how to reach you

We ask for this for two reasons. It lets us tell one person's account from several, which matters when the same name comes up more than once. And it's how we reach you if you asked us to. We will not share your name or contact information with anyone you're reporting.

The name you'd like us to use.
A name, so we can tell your account from others.
One way to reach you. Email gets you your case number right away.
Before you send*
I understand that what I send gets its own file and case number, that it's read by Revival's board, and that a report is information, not a verdict. Revival decides what to do inside its own policies and can't punish or police anyone beyond its own events and community. Sending this doesn't commit me to anything further.
Tick this so we know you've read it.

You'll get a case number at the contact you gave us.
